Admission Process
To initiate hospice services, a patient, family member, friend, or attending physician can simply call the hospice. A nurse will then visit the patient at their residence or hospital to present information about the program. If the patient chooses to enroll, the hospice team will handle all necessary admission arrangements. For more information on hospice care for terminal illnesses, contact the hospice directly: (747) 264-0770.
